Patient experience
Find workforce risks that can affect patient experience.
Your HCAHPS scores are a workforce report card. Communication with nurses, responsiveness of staff, discharge information — every domain traces back to how people on your units work and decide under pressure. Yet most improvement efforts target scripts and rounding checklists while the underlying fit problems stay invisible.
Scores are symptoms. Staffing patterns are causes.
Cognicit maps each role's decision patterns to the CAHPS domains they touch. When a unit's scores slip, you can see whether the cause is a staffing pattern — the wrong mix of decision styles, burnout risk concentrated on one shift, a new hire mismatched to the role's demands — instead of guessing. The point is not to blame staff for scores. It is to give leaders the workforce side of the story, so improvement plans address causes instead of symptoms.
- Assess the unit.Staff complete the role-specific assessment (about 45 minutes).
- Map to CAHPS domains.Decision patterns and work styles are mapped to the patient-experience domains each role influences.
- Spot the pattern. When scores move, compare them against the workforce picture: fit gaps, burnout concentration, team imbalance.
- Intervene precisely.Coaching, reassignment, or targeted hiring — directed at the actual pattern, not the whole unit.
What changes when you can see the pattern
- Experience initiatives stop treating every unit the same.
- New hires are screened against the decision demands of the roles that touch patients most.
- Burnout risk gets flagged before it shows up as a responsiveness score.
- Boards see the workforce story behind the patient-experience numbers.
Research keeps finding the same gap: what executives measure and what the workforce reports are two different pictures. In the 2026 Penn Nursing study of 410 hospitals, nurses' own reports of understaffing predicted longer ER stays and more patients leaving before being seen — effects invisible in bed counts (Penn Nursing, October 2026). Cognicit exists to make the workforce's picture visible to leadership before patients feel it.
The economics argue for looking earlier. Replacing one RN averages $61,000, and national hospital turnover runs 16.4% (NSI Nursing Solutions, 2025 NSI National Health Care Retention & RN Staffing Report). One-third to one-half of engagement differences trace to stable personality traits (Fukuzaki & Iwata, 2022; Young et al., 2018) — the layer engagement surveys do not measure. Cognicit measures that layer and links it to the scores behind reimbursement: CAHPS, HEDIS, and HOS.

What healthcare leaders ask us.
We already do patient-experience training. How is this different?
Training teaches skills. Cognicit reveals fit: whether the people in patient-facing roles are wired for the decisions those roles demand. Training cannot fix a mismatch it cannot see.
Which CAHPS domains does it map to?
The mapping covers the domains driven by staff behavior, including communication with nurses and doctors, responsiveness, and discharge information.
Do you need our HCAHPS data to start?
No. The workforce assessment stands on its own. If you share scores, the platform can correlate patterns with your results over time.
